Advices I would give to a patient in a pre-participation physical or check up would be to :

- don't hesitate to do routine consultation with a doctor really often, even if it seems that nothing is wrong. Better be safe than sorry ! 

- respect the limits advised by the doctor, even if you feel like you can do more

- think also about your mental health. You can hurt yourself not only physically but also mentally with burn-out for exemple, so take pauses and rest !

- regularly check on your co athletes in your team. Maybe you will spot something wrong, if not, it's always good to have a strong team spirit, and a team here for each other.

I would ask the questions :

How often do you train, and for how long ? 

Do you keep track of your diet, and do you need help to manage it with a nutritionist for exemple ?

Do you feel a light pain that doesn't seem serious but is here since a long time ? 

Did you change something in your training and practice recently ? Do you plan to in the future ?

Is your team and particularly your coaches open to talk to if you have any injuries ?

Did you feel a rise or fall in your performances lately ?
