First, I would tell the patient that it is very important to listen to their body. If they are feeling dizzy or foggy, it is safer for them to leave the game rather than stay on the field. Indeed, the accumulation of hard hits to the head can cause long-term neurological issues.

I would then ask them if this has ever happened to them: 'Have you ever felt dizzy, lightheaded, or experienced chest pain specifically while running or during heavy exertion?' Finally, I would ask a more mental-health-oriented question: 'Are you feeling any pressure to play through pain from coaches, parents, or yourself?'

I completely agree, I would also emphasize the importance of proper warm-up and protective gear to prevent injuries. you are right, it's really important to pay attention to what your body is telling you.
